In order for us to provide information about your file to another person, we need a written consent (power of attorney) from you.
You can send this to us either by mail or by e-mail. Our address and e-mail address can be found in our correspondence.
Please include your full name, your address, and your date of birth in the power of attorney, as well as the contact details of the person to whom you are granting the power of attorney.
The declaration must clearly state that the person concerned is allowed to receive information about your data held by us and is authorized to act on your behalf.
